Tratando a acondroplasia: o direito de cada um

Introdução

Todo dia é dia de aprender algo novo. Como o meu interesse na acondroplasia é sempre maior, tornei-me membro de um pequeno número de grupos relacionados a essa condição genética no Facebook, onde estou sempre aprendendo. Na maioria das vezes esses grupos, que mantêm discussões muito ricas ou simplesmente a troca de experiências de pais ou pessoas com nanismo, em que sabedoria é prevalente, também testemunham debates ásperos em relação a terapias atuais (cirúrgica) e potenciais (drogas em investigação) para acondroplasia. No entanto, enquanto há comentários sinceros e perguntas como por que devemos tratar de algo que é normal? ou ela (a criança) é perfeita como ela é, um presente de Deus são comuns, às vezes, também podemos ver outros cheios de raiva, com pensamentos preconceituosos como você quer extinguir a raça? (a pessoa que expressou esse pensamento estava se referindo a uma "raça de anões") Esse comentário, que eu li esta semana, foi o que me motivou a publicar este texto.

É sobre quem somos

Há apenas uma raça, a raça humana. Com toda a sua diversidade de pacotes de apresentação. Muito, muito frequentemente, o pacote não é perfeito, mas as imperfeições vêm em uma ampla gama de graus de gravidade. Às vezes é apenas um hemangioma na pele, ou uma visão ruim, porque a lente do olho está torta e você não consegue ver longe (miopia). Raramente, a imperfeição é tão grave que não é compatível com a vida, como na anencefalia. Além disso, a imperfeição pode aparecer mais tarde na vida (e elas também são extremamente comuns).

Os seres humanos vêm evoluindo e aprendendo a lidar, manejar, tratar e mais e mais, curar essas imperfeições. Há muito tempo atrás, os óculos foram inventados para corrigir alguns dos problemas oculares (Figura 1) e a lente telescópica foi a grande ferramenta que ajudou Galileu a desenvolver sua teoria heliocêntrica. Mas agora, não só existem milhões de óculos da moda e o Hubble mas, também para tratar (e curar) miopia e outras condições do olho, a cirurgia corretiva já está muitas vezes disponível. Ou você pode simplesmente pingar gotas nos olhos para tratar o glaucoma.

O que alguém poderia dizer a um indivíduo que define invenções ou inovações ou soluções que visam o bem estar do ser humano (neste caso particular, para a acondroplasia) como uma ameaça para a raça?

No século passado, organizações fortes e respeitáveis ​​cresceram em defesa dos direitos sociais e têm lutado arduamente para melhorar a vida das pessoas afetadas por doenças genéticas que levam à restrição do crescimento. Elas surgiram com o aparecimento de outras organizações sociais que defendem os direitos de outras minorias. Estas organizações têm lutado uma longa batalha para estabelecer benefícios para aqueles que têm de lidar com mais desafios na vida diária. Muitas décadas passaram para os americanos de origem africana até obterem os mesmos direitos sociais que outros americanos tinham. A Sociedade agora tem mais consciência sobre a diversidade da raça humana.

Então, quando um indivíduo vem e diz que terapias para o nanismo são uma ameaça para a raça, algo está muito errado. Esse tipo de argumento vai contra tudo o que foi alcançado por estas respeitáveis organizações.

O Estado de Saúde

Um ano atrás, pensando nos debates sobre porque devemos tratar acondroplasia ou qualquer outra condição ou doença que traz sofrimento para o indivíduo afetado, publiquei o texto a seguir no grupo Acondroplasia, no Facebook. Sofrimento aqui é definido como qualquer tipo de desafio enfrentado de forma desequilibrada por um indivíduo afetado, de um modo que alguém que não é afetado por essa condição não o teria.
Tem havido muita controvérsia em torno de opções terapêuticas para a acondroplasia e outros tipos de nanismo ultimamente. O alvo hoje são intervenções cirúrgicas, tais como alongamento de membros, o que é uma opção viável para aqueles que decidem por ele.
A cirurgia de alongamento não elimina a mutação genética. Assim, isto não seria o caso de levar ao desaparecimento da diversidade humana. Também não seria capaz de corrigir todos os problemas de saúde que podem ocorrer em alguém com acondroplasia. No entanto, a cirurgia pode corrigir alguns deles, como a lordose lombar que pode piorar a frequente estenose da coluna espinhal, e as pernas em arco, que também são causa de pressão excessiva nos joelhos, com consequências conhecidas, entre outros benefícios médicos.
No contexto de atacar publicamente aqueles que decidem por fazer estas intervenções cirúrgicas, aspectos sociais e psicológicos têm sido também frequentemente subestimados e são colocados juntos com o aspecto estético para reduzir a sua relevância.
O estado de saúde é mais do que apenas não estar doente, como foi definido há quase 70 anos pela Organização Mundial da Saúde:
"A saúde é um estado de completo bem-estar físico, mental e social e não meramente a ausência de doença ou enfermidade". 
É um direito individual de decidir ou não por um caminho ou outro. Por exemplo: pode-se decidir se ele / ela vai fazer uma cirurgia para correção de miopia ou continuar a usar óculos. Pode-se optar por implantar próteses de mama depois de uma cirurgia de câncer de mama. Pode-se argumentar que ambos os procedimentos acima são cosméticos em primeiro lugar, mas na verdade eles estão mirando necessidades que vão além do mero estado físico de saúde.
Pode-se não concordar com uma decisão de um outro indivíduo, e este é também um direito de qualquer indivíduo. Mas por que é que alguém ataca agressivamente algo que pode trazer muitos benefícios (reais ou imaginários) para o outro?
Terapias para acondroplasia e outras formas de nanismo será uma realidade na próxima década, e isso é bom.
Hoje, o alvo é a cirurgia de alongamento. Amanhã, ele pode se tornar uma terapia farmacológica. Ambas as abordagens podem ser entendidas como visando um objetivo só: melhorar a Saúde do indivíduo, nos termos gerais da definição da OMS. 
 Pense nisso.

Treating achondroplasia: just one's right

Introduction

Everyday is a day to learn something new. As my interest for achondroplasia grows stronger, I have joined a small number of groups related to this genetic condition in Facebook, where I keep learning a lot.These groups, most of the time hosting very rich discussions or simply the sharing of experiences of parents or individuals with restricted growth conditions, in which wisdom is prevalent, also witness harsh debates regarding current (surgical) and potential (drugs in investigation) therapies for achondroplasia. Nevertheless, while sincere comments and questions such as why should we treat something that is normal? or she (the kid) is perfect as she is, a gift of God are common, sometimes we can also see others full of rage, bearing prejudiced thoughts like do you want to extinguish the race?  (the person who expressed this thought was referring to a "dwarf race") Believe me, this one I read this week and is the one which motivated me to publish this text.

It is about who we are

There is just one race, the human race. With all its presentation package diversity. Very, very frequently, the package is not perfect, but the imperfections come in a wide range of severity levels. Sometimes it is just a hemangioma in the skin, or a bad vision, because the eye lens is distorted and you cannot see far away (myopia). Rarely, the imperfection is so severe that it is not compatible with life, like in anencephaly. In other cases, the imperfections will appear later in life (and they are also extremely common).
Humans have been evolving and learning how to deal, manage, treat and more and more, cure these imperfections. Long time ago, glasses have been invented to correct some of the eye issues (Figure 1) and magnification was the great tool for Galileo to develop his heliocentric theory. But now, not only there are millions of fashionable glasses and the Hubble, but also to treat (and cure) myopia and other eye conditions, corrective surgery is often available. Or you can just drip drops in your eyes to treat your glaucoma.

What could someone say to an individual that defines inventions or innovations or solutions aiming the human well being (in this particular case, achondroplasia) as a threat to the race?

In the last century, strong and respectable organizations have grown defending social rights and have been fighting hard to improve the life of individuals affected by genetic conditions that lead to growth restriction. They came together with the raise of other social organizations which defend other minorities' rights.  These organizations have been fighting a long fight to establish benefits for those that have to handle more challenges in the daily life. It took many decades for Americans of African origin to gain the same social rights other Americans had. Society now has more awareness about the diversity of the human race. 

So, when someone comes and says that therapies for dwarfism are a threat to the race, something is really wrong. This kind of argument goes against all that has been achieved by these renowned organizations.

The state of Health

One year ago, thinking about those debates about why should we treat achondroplasia or any other condition or disease that brings suffering to the individual affected, I have published the following text in the Achondroplasia group in Facebook. Suffering here means any kind of challenge one may face in an unbalanced manner, in a way that someone who is not affected by that condition would not. 

There has been a lot of controversy around therapeutic options for achondroplasia and other types of dwarfism lately. The target today are surgical interventions such as limb lengthening, which is a feasible option to those who decide for it.
Limb lengthening would not eliminate a gene mutation. So, this would not be the case of vanishing the human diversity. It would also not correct all medical issues that may occur in someone with achondroplasia. However, it may correct some of them, such as the lumbar lordosis that may worsen the common spinal stenosis and the common varu legs, which also are cause of undue pressure on knees, with known consequences, among other medical benefits.
In the context of publicly charging against those who decide to go for these surgical interventions, social and psychological aspects have been also frequently underestimated and are put together with the cosmetic aspect to reduce their relevance.
The state of Health is more than just not being ill, as has been defined almost 70 years ago by the World Health Organization: 

"Health is a state of complete physical, mental and social well-being and not merely the absence of disease or infirmity."

It is an individual right to decide or not for a path or another. For example: one can decide if he/she will have a surgery for myopia or continue to use glasses. One can decide to have breast prosthesis implanted after a surgery for breast cancer. One can argue that both above procedures are cosmetic in first place, but in fact they are targeting needs that go beyond the mere physical state of health. 
One may not agree with a decision by another individual, and this is also one's right. But why does one fight so hard against something that may bring many benefits (real or imagined) for the other? 
Therapies for achondroplasia and other forms of dwarfism will be a reality in the next decade and this is good. 
Today, the quest is surgery. Tomorrow, it may become a pharmacological therapy. Both approaches may be understood as aiming one goal only: to improve the Health of the individual, in the broad terms of the WHO definition.
Think about it.
